EMPLOYEE CRUSHED UNDER VEHICLE DURING MAINTENANCE

EMPLOYEE #1, A WRECKER/MECHANIC, WAS PERFORMING ROUTINE MAINTENANCE ON A WRECKER WHILE IT WAS SUPPORTED BY A NAPA HYDRAULIC 2-TON FLOOR JACK. NO OTHER MEANS OF SUPPORT WAS BEING USED. TOOLS FOUND BENEATH THE WRECKER WERE OF THE KIND USED IN TIGHTENING "U" BOLTS ON FRONT WRECKER SUSPENSIONS. EMPLOYEE #1 WAS LAYING PARTIALLY BENEATH THE WRECKER BUMPER ON HIS RIGHT SIDE ON THE ROLLER CREEPER. HIS LEGS WERE EXTENDED TOWARD THE JACK HANDLE. HE ATTEMPTED TO LIFT THE WRECKER HIGH WITH THE JACK, USING HIS LEG TO PUMP THE JACK HANDLE. THE JACK HANDLE INADVERTENTLY TURNED, RELEASING THE HYDRAULIC PRESSURE IN THE JACK ASSEMBLY. THE WRECKER CAME DOWN AND PINNED EMPLOYEE #1 BETWEEN THE BUMPER AND ROLLER CREEPER, KILLING HIM. WITH THE WRECKER BODY ON THE GARAGE FLOOR, THE CLEARANCE BETWEEN THE CREEPER AND THE WRECKER'S BUMPER WAS 8 1/2 INCHES.
