Employee injured when struck by forklift falling from jack

To change the oil, Employee #1 placed a hydraulic jack under the lower right edge of a 9,300 lb forklift. With his upper body under the forklift, the jack slipped out and the forklift fell, crushing Employee #1. He suffered head injuries and was hospitalized.
