LOST BALANCE AND FELL INTO OPERATING MACHINERY

ON JULY 30, 1985, AT ABOUT 1:40 PM, EMPLOYEE #1 WAS CUTTING OFF THE END OF A 3/4 INCH BOLT. THE BOLT SECURED THE AUTOMATIC KNOCK-OFF RING TO THE REAR FRAME OF A RIGID-FRAME STRANDER. HE WAS USING A RIGHT-ANGLE GRINDER WITH A NORTON LAMINATED GRINDING WHEEL. WHEN THE BOLT WAS WITHIN ABOUT 1/8 INCH OF BEING CUT THROUGH, THE WHEEL JAMMED. THE GRINDER JUMPED, THROWING EMPLOYEE #1 OFF BALANCE. HE FELL INTO A TUBULAR STRANDING MACHINE WHICH WAS LOCATED 7 FEET BEHIND HIM. THE STRANDER WAS IN OPERATION AT THE TIME. HE RECEIVED SEVERE HEAD INJURIES, AND DIED IN THE HOSPITAL THE FOLLOWING DAY AT ABOUT 3:30 AM.
