ELECTRIC SHOCK - DIRECT CONTACT WITH ENERGIZED PARTS

Three employees were rebuilding the wooden portions of a large billboard. They were working from 6.1-meter-high (20-foot-high) scaffolds and on 10.7-meter-high (35-foot-high) stringers. There were no guardrails or safety lines protecting the workers. The employees had not opened the disconnect for the electrical system but were relying instead on a timer and an electric eye. The circuit became energized intermittently either due to a fault or vibration. One employee apparently contacted energized parts and received an electric shock. One of his coworkers tried to lower the injured employee to the ground with a rope. Unfortunately, the coworker could not hold the weight, and the injured employee fell to the ground. The injured employee died of electrocution.
