Employee crushed and killed by truck bed

Employee #1 was changing the lift control cable on a ten-wheeled dump truck, #17-950. The new cable was installed on the control lever but had not yet been connected to the control valve on the pump. The dump bed was elevated about 18 inches when Employee #1 apparently leaned in over the left side of the frame and activated the control valve lever, causing the bed to fall. Employee #1 was killed. He was working alone and there were no witnesses to the accident.
