Investigation Summary
At 12:30 p.m. on January 11, 2021, an employee was waiting for one coil to tail off to start the second coil. The employee noticed a piece of tape on the top inch roll and attempted to remove the tape. As he attempted to remove the tape, his hand was pulled into the bite of the pinch rolls. The employee suffered a crushing injury to his right hand and forearm and was hospitalized.
