Four employees were working at a millwork firm. One worked as a machine operator . On August 12, 2020, the employer contacted the Division to report that four of their employees had tested positive for COVID-19 and were hospitalized. On Augu st 13, 2020, the Division opened an investigation with the employer. On August 3 1, 2020, the employer contacted the division to report that one of the hospitali zed employees had died from exposure to COVID-19.
