Summary Nr: 128019.015Event: 07/15/2020Employee Amputates Both Legs When Hit By Forklift
At approximately 6:25 a.m. on July 15, 2020, an employee opened the cylinder. T he coworkers and the employee was walking along the tram area with a twoway rad io in his hand to open cylinder 2. The employee then proceeded to walk towards C ylinder 2 controls to move the cart and open the door to Cylinder 2 when an onco ming forklift that was returning in reverse to continue unloading the tram, stru ck him from behind. The employee who lost both legs in this forklift accident wa s hospitalized.
