Investigation Summary
At approximately 6:25 a.m. on July 15, 2020, an employee opened the cylinder. The coworkers and the employee was walking along the tram area with a twoway radio in his hand to open cylinder 2. The employee then proceeded to walk towards Cylinder 2 controls to move the cart and open the door to Cylinder 2 when an oncoming forklift that was returning in reverse to continue unloading the tram, struck him from behind. The employee who lost both legs in this forklift accident was hospitalized.
