FALL FROM ROOF

EMPLOYEES WERE INSTALLING FIBERGLASS INSULATION AND .24 GAGE GALVANIZED SHEETING ON A ROOF THAT HAD A 1/12 PITCH. ROOF MEASURED 100' IN LENGTH AND 40' IN WIDTH. THE HEIGHT FROM GROUND LEVEL TO THE EAVES WAS 16', AND 20' TO THE RIDGE. THERE WERE A TOTAL OF 5 WORKERS ON THE ROOF. THEY WERE ENGAGED IN SETTING THE 100' X 4' SECTIONS OF FIBERGLASS INSULATION IN PLACE AND INSTALLING THE SECTIONS OF GALVANIZED SHEETING (24'-20' IN LENGTH AND 39" IN WIDTH) ON TOP OF THE INSULATION. THE EMPLOYEES WERE APPROX. 2-3' FROM THE SIDE WHEN SETTING THE NEW INSULATION AND SHEETING. THEY WOULD WORK FROM THE NEW SHEETINGTHEY HAD JUST LAID. EMPLOYEE #1 WAS WORKING AT THE SOUTHEAST SIDE OF THE BUILDING AT THE EAVE INSTALLING A 3 1/2 " ALUMINUM BAND SEALER ON THE EAVE. HE WAS CHECKING TO MAKE SURE THE BAND WAS SECURED PROPERLY WHEN HIS LEFT FOOT CAUSED THE SHEETING TO BEND. HIS FOOT THEN SLIPPED TO THE OPEN SIDE APPROX 2' AWAY. HE LOST HIS BALANCE AND FELL 16' TO THE CONCRETE FLOOR BELOW. DURING THE FALL HE STRUCK THE SAG RODS THAT WERE SUPPORTING THE GIRDERS. THIS CAUSED HIM TO FALL BACK DURING THE FALL AND LAND ON HIS BACK. HE WAS THEN TRANSPORTED BY HELICOPTER TO PENNINSULA GENERAL HOSPITAL IN SALISBURY AND ADMITTED TO INTENSIVE CARE WITH 2 BROKEN RIBS, A MILD CONCUSSION, MULTIPLE BRUISES TO THE HIP AND ELBOWS, A PUNCTURED LUNG AND A LACERATION TO THE HEAD REQUIRING 9 STITCHES. HE WAS WEARING CANVAS TENNIS SHOES WHEN THE ACCIDENT OCCURED. HE INDICATED A LIGHT LUBRICANT HAD BEEN APPLIED TO THE GALVANIZED SHEETING THAT HE HAD BEEN WORKING ON TO PREVENT RUSTING. NO PERSONAL PROTECTIVE EQUIPMENT WAS WORN AT THE TIME OF THE ACCIDENT. EMPLOYEE WAS RELEASED FROM THE HOSPITAL ON 6/28/84.
