CRUSHED BETWEEN PARTS OF LUMBER STACKING MACHINE

EMPLOYEE #1, AT AN EXPOSED POSITION IN A "LAWSON STACKER" (LUMBER HANDLING EQUIPMENT), TOLD A CO-WORKER TO START THE MACHINE TO FACILITATE ITS OILING. WHEN THE MACHINE STARTED, HE WAS CAUGHT AND CRUSHED BETWEEN MOVING PARTS. EMPLOYEE #1 WAS A MATERIAL HANDLING LABORER/HELPER. THE MACHINE OPERATOR WAS ABSENT. NO SPECIFIC WORK PROCEDURES OR RULES HAD BEEN ESTABLISHED FOR OILING THE MACHINE. NO TRAINING INSTRUCTIONS HAD BEEN GIVEN TO THE HELPERS.
