At 4:00 p.m. on April 27, 2020, Employee #1, employed by an elevator company, was installing a new hydraulic elevator at a multiemployer construction project, a two-story building that was being renovated. As he climbed a portable extension ladder to hang hoisting equipment, the ladder's feet slipped. The employee fell down the elevator shaft and struck the concrete surface, a fall height of 32 feet. Employee #1 sustained multiple fractures and was killed. | ||||||
