Investigation Summary
At 5:00 a.m. on March 11, 2019, an employee was filing large boxes with plastic pelletized material from a large mixing drum. The employee reached into a discharge chute to clear material from the opening while the mixing paddle inside the drum was running and caught his right hand's middle finger. The employee amputated his finger and was hospitalized.
