Summary Nr: 114036.015Event: 02/28/2019Employee Is Crushed And Killed By Falling Pallet Jack
At 10:15 a.m. on February 28, 2019, an employee was repairing a pallet jack that was elevated by way of a 7 ton jack stand. During work, the pallet jack slid from the jack and struck the employee. The employee was crushed and killed. The incident investigation identified that there was no blocking or bracing in place.
