Investigation Summary
At 8:45 a.m. on February 15, 2019, an employee was working for a network of hospital-affiliated physicians and mental health specialists. He and a coworker were working on a pulley and drive belt assembly for a large building exhaust fan. They were replacing the three drive belts. According to the coworker, the employee pulled out the blocking bar that had been inserted into the pulley to keep the fan from moving from ambient air sources. The employee somehow got his hand inside the path of the drive belts and nip point at the pulley. According to the employee, it was the coworker who pulled out the blocking bar. Either way, as the employee was trying to reinsert the bar, his left hand became caught in the nip point. He sustained the amputation of the index finger on his left hand down to the first knuckle and fractures or a fracture of the small finger on his left hand. He was not hospitalized.
