Investigation Summary
At 11:30 a.m. on December 11, 2018, an employee was operating a forklift to lift a bulk bag of seed over a hopper before dumping its contents. The employee was passing a knife to a coworker through the forklift's mast when his knee came into contact with the control lever and lowered the mast. The employee fractured his left arm when it was caught by the mast as it lowered. The employee was hospitalized.
