At 12:39 p.m. on February 4, 2019, an employee was standing on a flatbed trailer and was assisting a coworker who was using a forklift to load a 20 foot steel pipe. The employee was crushed and killed from blunt force trauma when he fell from the trailer and the pipe rolled off of the trailer and landed on him.
