WHILE THE WELL TUBING WAS BEING PRESSURIZED FOR LEAKS, THE COUPLING ON THE TUBING RUPTURED. THE RESULTING FRAGMENTS STRUCK EMPLOYEE #1 IN THE CHEST, FATALLY INJURING HIM.
