Investigation Summary
At 8:30 a.m. on November 23, 2018, an employee was cleaning around auger pits inside the machine without locking it down. The employee's gloved right hand was caught by the rotating auger and pulled into the bits. The employee amputated four fingers below the knuckles and his thumb was amputated above the knuckle.
