Investigation Summary
At November 11, 2018, an employee was stationed at Section B of an Air Heater Unit during a pressure cleaning operation at a generating plant. The confined space entry door was open during the operation, and he entered into the air heater. The employee radioed to other employees to shut down the operation. However by the time the unit was shut down, the employee was located inside the air heater unit unresponsive.
