At 6:00 a.m. on November 13, 2018, an employee and a coworker were cleaning a wi cket machine. The employee was wiping down the machine with a rag and alcohol w ithout locking the machine out. The employee wrapped the rag around his hand an d the rotating roller caught his hand, partially amputating his right thumb.
