Investigation Summary
At 5:30 a.m. on November 5, 2018, Employee #1 was cleaning the area around the work station. The employee finished cleaning the area around his work station. While doing so, the employee walked by a rotating conveyor pulley which was unguarded. The employee's glove was grabbed by the conveyor pulley and belt pulling his arm into the conveyor. The machine automatically shut down and the employee was freed from the machine. The employee was hospitalized overnight in the emergency room for a broken left arm. The employee was working alone at the time of the incident and weather did not play a factor in the incident.
