At 9:00 a.m. on September 5, 2018, Employee #1, employed by a technology products and services company, noticed the width of the paper was incorrect in Press #2. He removed the paper and proceeded to load the correct new roll of paper onto Press #2. Employee #1 placed a core roll behind the new roll of paper so the paper would align with the feed. He hit the lever to raise the roll holder. As Employee #1 was pushing the roll of paper onto the holder, he placed his hand onto the hydraulic piston to get leverage. The hydraulic piston moved up, pinching Employee #1's right index finger between the head and body of the piston. His right index fingertip was amputated.
