Accident Investigation Summary
Summary Nr: 107708.015Event: 07/26/2018Employee Is Killed By Fall Caused By A Heart Attack
At 3:00 a.m. on July 26, 2018, an employee was installing a new LED light fixtur e from the ceiling. The employee used a scissor lift to access the area then ex ited the lift to perform the wiring portion of the task. As this task was being performed from an elevated location without the benefit of fall protection, the employee fell 23 feet below and struck a laundry conveyor member and ultimately the ground level below. It was later determined by the Lucas County Coroner th at the deceased suffered a heart attack at some point while performing the wirin g task in this condition.
