Investigation Summary
At 6:30 p.m. on March 13, 2018, an employee was troubleshooting a palletizer machine. The machine had been stopped and the employee had entered the interlocked safety gate to diagnose the problem. He leaned into the palletizer to remove a jammed piece of broken pallet and in doing so, blocked the pallet sensor for more than three seconds. This sensor block then triggered the machine to lower the pneumatic pallet stop device and support bracket, resulting in the amputation of the employee's left pinky finger tip.
