Investigation Summary
At 5:10 a.m. on March 20, 2018, an employee was removing the banding on a pallet of glass sheets. The pallet was holding 255 vertically stacked glass sheets weighing 4,623 pounds. The employee was caught between the glass sheets and load backrest of a forklift when the glass sheets shifted, killing the employee from blunt force trauma to his head.
