Investigation Summary
At 9:00 a.m. on March 19, 2018, an employee was assessing a jammed polyester fiber carding machine (separator) with coworkers. The machine was jammed with excess polyester fiber. After the machine was turned off and the rotating drum had stopped, a coworker opened a guard of the carding machine. During the assessment, a coworker turned on both machine motors (chain/sprockets motor and belt/pulleys motor) by depressing the start buttons on the control panel. After observation, a coworker turned off the chain/sprockets motor by depressing the proper stop push button on the control panel. Later, the employee turned off the belt/pulleys motor by depressing the proper stop push button on the control panel. The employee opened the knife switch (isolation device) that provided electrical energy to the control panel. A lockout device was not applied to the knife switch. After 10 seconds elapsed, the employee placed his hand inside the carding machine. The hand and arm were caught-in the ingoing nip-point created by the rotating drum and stationary worker stripper rollers. The employee's arm was dragged into the machine and was amputated.
