Investigation Summary
At 1:00 p.m. on March 15, 2018, an employee was installing an electrical box in a ceiling of a residential home and was working from a 2 foot step ladder that was on a scaffold plank that spanned across a stairway that led to the second floor. The employee fell approximately 12 feet when a board that was nailed into some studs gave way and the employee was killed.
