At 1:45 p.m. on June 8, 2018, Employee #1 and a coworker were using one lockout device to troubleshoot a knitting machine. The coworker removed the lock and energized the machine without realizing that Employee #1 had his fingers between gears. This resulted in an amputation to Employee #1's left index finger.
