At 4:10 p.m. on January 23, 2018, an employee was pressure testing valve assembl ies in a water test tank. The employee was placing a valve assembly into the te sting fixture. The employee reached for the lever that operates the side testing head and inadvertently activated the back testing head. The employee's left mi ddle and index finger was between the valve assembly and the back testing head. The employee's fingers were caught between the test head and valve assembly whe n the test head moved forward. The tip of the employee's left middle finger was amputated and his left index finger was fractured. water test tank testing fix ture. The employee was not hospitalized.
