At 6:45 a.m. on November 28, 2017, Employee #1 was hoisting a sea train onto the bed of a trailer. The employee was standing adjacent to a low bed trailer while operating the trailer winch line controls. A 0.375 inch Campbell Grade 70 chain, which failed during the operation, struck the employee in the head. Employee #1 sustained a head laceration and a skull fracture, and was hospitalized. | ||||||
