Investigation Summary
At 4:50 a.m. on January 4, 2018, an employee was using a Raymond standup forklift to stow pallets of paper product in the pallet rack bays of a warehouse. After just placing a pallet in the racking bay and was backing out of the bay to pick up another pallet, the employee's fright foot became caught between the forklift and a pallet rack. The employee's right heel sustained a laceration, which he was hospitalized.
