Investigation Summary
At 4:45 a.m. on December 29, 2017, a warehouse employee was unloading merchandise using a Raymond, Model 112TM-FRE60L powered rider pallet jack. The employee was looking towards their right side instead of the direction of travel when they crashed into a wall resulting in a fracture to their right ankle. The employee was hospitalized for more than 24 hours for the injury.
