At 8:30 a.m. on November 15, 2017, an employee was assisting a vendor with the setup of a new machine (KBP Roll Forming Machine; serial #2177014). The employee was not familiar with this machine and the machine was not guarded. The employee's hand was crushed when the rack and pinion moved, amputating his finger.
