Employee killed in fall off rear of scaffold

Employee #1 was making repairs to a semi-trailer. He placed a scaffold at the north side of the semi- trailer. Employee #1 climbed onto the scaffold and most likely fell off the rear of the scaffold. There was no standard top or mid-rails on the scaffold. Employee #1 struck the right side of his head and body on the concrete. Employee #1 was hospitalized and released on January 19, 2001. He reentered the hospital on January 20, 2001 and died.
