Employee #1 died from a blow to the head

On May 6, 2001, Employee #1 was operating a payloader that got tangled in an airline hanging from a crane and pulled it from the plant airline that had 90. The crane airline and quick disconnect that had been pulled from the plant airline, went through the rear window of the payloader. The metal quick disconnect struck the victim behind the left ear resulting in a fatal blow to the head.
