Investigation Summary
Employee #1 was on top of a 12 ft 9 in. ft tall scaffold platform retrieving a hammer when she apparently lost her footing. She fell into the 19 to 20 in. wide gap between the scaffold and the wall and dropped to the ground. Employee #1 was killed.
