Employee killed when crushed against wall by backing truck

Employee #1 was using a forklift to load a tractor trailer when he noticed the trailer moving. He dismounted, went to the rear of the trailer, and stuck his head out around the back end on the driver's side to tell the driver that he was loading it. The driver, who was apparently trying to engage the truck and trailer together, started to back up, catching Employee #1's head between the left side wall of the trailer and the concrete wall. He died at the scene a few minutes after the accident.
