Investigation Summary
At 3:30 p.m. on October 15, 2017, Employee #1 was changing shifts at a fast food restaurant, when he and his Shift Leader started horseplaying and wrestling at the food preparation area, located behind the food serving area. The employee slipped and fell backwards, hitting the back of his head on the tile floor. Employee #1 was unresponsive and had to be admitted to the local hospital for a skull fracture.
