Investigation Summary
On January 19, 1999, Employee #1 and other employees were installing 6-in. storm piping in an excavation site. The excavation site had a depth of 3.5 ft and located about 4 ft from the nearby building. During the pipe installation process, a large section of frozen clay from the building side slide down and pinned Employee #1 to the wall of excavation site. The other employees removed the debris upon Employee #1 and freed him. He sustained minor injuries in the accident.
