Employee falls from ladder when struck

On March 25, 1999, Employee #1 was stripping upper deck forms. A form beam came loose and struck Employee #1 in the head. He was knocked off the step ladder and fell 3 feet. Immediately following, another form beam came loose and fell on Employee #1. Employee #1 suffered from a laceration and was hospitalized.
