Employee killed when crushed in machine

Employee #1 was performing maintenance on a machine system. He entered the machine by opening a guard and climbed onto the shuttle conveyor that fed the machine. He did not shut off and lockout the system. A coworker, the machine operator, did not know Employee #1 was in the machine and activated the machine causing the shuttle to advance, crushing Employee #1 in the pinch area between the shuttle and stationary parts of the machine. Employee #1 was killed.
