Investigation Summary
Employee #1 was helping a coworker clean the inside a concrete mixer. For some reason, Employee #1 climbed back into the mixer as the coworker was starting it. Employee #1 was crushed and killed by the rotating mixer blades. He was pronounced dead at the scene.
