BURN - ELECTRICAL FAULT IN FUSE BOX

One morning a welding supervisor was restoring power after an unexpected outage at his workplace. He pulled the fuses for two disconnects from a fuse box and then reinstalled them. As he was reinstalling them, an electrical fault occurred. The ensuing electric arc ignited the employee's clothing. He died 2 weeks later due to complications from second- and third-degree burns.
