Investigation Summary
At 10:00 a.m. on October 5, 2017, an employee was operating a labeling machine which became jammed in the feeder rolls. The employee attempted to remove the jam by grabbing the label from the rollers to pull it out and caught his glove and finger. The employee amputated his right middle finger and was hospitalized.
