Employee killed in fall onto iron rod in excavation

Employee #1 was walking at the edge of an 8 ft in diameter by 4 ft deep excavation. He fell into the excavation and onto a 5/8 in. iron rod that penetrated the right side of his groin. Employee #1 died two hours later at the hospital.
