Employee's fingers amputated in cotton swab machine

Employee #1, a trainee, was doing preventive maintenance on a cotton swab machine under the direction of the lead maintenance trainer. He was using compressed air to blow out the machine to remove excess cotton fibers in and along the chain system. The compressed air tip and his left middle and index fingers became caught between the sprocket and chain, and were amputated. Surgical attempts to reconnect Employee #1's fingers were unsuccessful. All of the machine's guards had been removed and the lockout function had been overridden before the accident.
