Employee killed when crushed between trailers

Employee #1 was cleaning the rear corner of a trailer from a ladder stand situated parallel to the rear bumper. Two coworkers were maneuvering a second trailer to the right of and parallel with him. One of the coworkers was pushing the second trailer on an axle dolly when it came off the dolly and fell toward the first trailer. Employee #1's head was crushed between the two trailers and he fell approximately 7 1/2 ft from the ladder stand onto concrete. He was killed.
