Employee's fingers amputated while training coworker

Employee #1 was providing on-the-job training for a new coworker. The coworker improperly placed a base into the crimp fixture, and Employee #1 reached around the light curtain to realign the base. The machine cycled and the first two fingers of his left hand became caught between the base and the chamber. They were crushed and amputated between the first and second joints.
