Employee killed in fall from top of building

Employee #1 was part of a crew working on top of a five-story building. The crew was setting a load of heavy angle irons, and some decking had to be moved before the load could be landed. A steel cable was strung 6 ft from the edge as both a lifeline that employees could tie off to, and as a warning bearer to remind them of their proximity to the edge. Employee #1 was moving the decking when he either stepped or fell outside the steel cable line and went over the edge, falling 80 ft to the ground. He was killed. Two witnesses stated that Employee #1 was pulling or pushing the sheets when he lost his balance. Another witness said that he was standing upright just before he fell. The company policy was for employees to always tie-off when outside of the warning line.
