Investigation Summary
At 6:30 a.m. on February 10, 1998, Employee #1, a set up mechanic, was cleaning off high tension rollers on the Warehouse #2 and Line #2 fabric softener machine with a hand held pad. The rollers were in motion. His right index finger got caught in the rollers and was amputated up to the middle joint.
